About Us
TotalCare Assisted Living Services is a model for the medically complex individuals with mental illness, or brain injury residing in a nursing home, hospitals and other facilities transitioning back to the community requiring some support services.
The program is managed by a registered pharmacist with a focus on medication management as a means to improve outcome.
The strength of this program lies in the design of small setting that fosters a more social and family atmosphere.
We are licensed by Minnesota Department of Health as an Assisted Living facilities..

Our team is experienced across a wide variety of methods designed to focus on your specific mental health as well as physical needs. We understand no two residents are the same and provide personalized treatment plans aimed at each person’s specific needs.
